Responsibilities:

• Responsible for assisting the patient with assigned tasks according to the care plan
• Bathing, Grooming, Dressing (includes braces and prosthesis)
• Skin care and nail care (excludes cutting of nails)
• Ambulation/transfer assistance
• I&O monitoring
• Range of motion exercises
• Vital Signs and weights
• Catheter and bowel care
• Meal preparation and assisting with dietary and nutritional needs
• General light housekeeping, laundry, and shopping specific to patient
• Observes and reports significant changes in patient's condition to supervisor
• Completes all documentation upon completion of shift
• Processes time according to policy
• Turns in documentation on a weekly basis
• Accompanies the patient to medical providers
• Maintains a mature and responsible relationship with staff, patients, and families
• Assists with wheelchairs and other medical equipment and devices (non-life support)

Job Requirements

• High School graduate preferred or equivalent of education and training
• Current CPR Certification
• Registration or Certification as required by state law and regulation
• Ability to follow oral and written instructions
• Ability to keep simple records
• Pediatric and Adult experience as appropriate for the patient

Physical Requirements

• Frequent lifting and repositioning of patients
• Repeated or prolonged standing, bending, kneeling, twisting and occasional climbing of stairs
• Acute sense of sight and hearing (corrected or uncorrected) and strong sense of touch, smell and taste
• Agility and strength sufficient to handle patients and equipment without assistance
• Must be able to appropriately respond physically and psychologically to emergency situations in the home or during transport
• Must be able to function in a wide variety of environments which may involve exposure to allergens and other health conditions
