Responsibilities: - Responsible for assisting the patient with assigned tasks according to the care plan
- Bathing, Grooming, Dressing (includes braces and prosthesis)
- Skin care and nail care (excludes cutting of nails)
- Ambulation/transfer assistance
- I&O monitoring
- Range of motion exercises
- Vital Signs and weights
- Catheter and bowel care
- Meal preparation and assisting with dietary and nutritional needs
- General light housekeeping, laundry, and shopping specific to patient
- Observes and reports significant changes in patient's condition to supervisor
- Completes all documentation upon completion of shift
- Processes time according to policy
- Turns in documentation on a weekly basis
- Accompanies the patient to medical providers
- Maintains a mature and responsible relationship with staff, patients, and families
- Assists with wheelchairs and other medical equipment and devices (non-life support)

Job Requirements
- High School graduate preferred or equivalent of education and training
- Current CPR Certification
- Registration or Certification as required by state law and regulation
- Ability to follow oral and written instructions
- Ability to keep simple records
- Pediatric and Adult experience as appropriate for the patient
Physical Requirements
- Frequent lifting and repositioning of patients
- Repeated or prolonged standing, bending, kneeling, twisting and occasional climbing of stairs
- Acute sense of sight and hearing (corrected or uncorrected) and strong sense of touch, smell and taste
- Agility and strength sufficient to handle patients and equipment without assistance
- Must be able to appropriately respond physically and psychologically to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other health conditions
