The RN is responsible for assessing, planning, directing, coordinating, evaluating and providing the patient care given to a specific group of patients utilizing the nursing process and established Hospital policies and procedures. Coordinates with the physician, other health care team members and family members to ensure the highest standard of care.

Specific Responsibilities:

  • Develops, implements and updates a comprehensive patient plan of care to meet the patient's physical, psychological and social needs
  • Performs holistic assessment on assigned patients with appropriate documentation
  • Assesses and manages needs and/or changes in patient's condition that are clinically relevant
  • Initiates and implements patient plan of care reflective of identified problems/needs, including desired patient outcomes and discharge plans
  • Implements the plan of care with actions that provide for patient and family participation in health promotion, maintaining and restoring the optimal level of health obtainable for the individual 
  • Works with physician, other health care team members, case management and/or the patient's insurance company regarding appropriate patient care issues
  • Develops, implements and evaluates teaching plan for patients and families
  • Identifies discharge planning needs of the patient early and coordinates with discharge planner appropriate referrals
  • Responsible for continuous and accurate patient care
  • Documents on a permanent record the patient's status
  • Administers medications and treatments safely with knowledge of their indications
  • Demonstration of the ability to interact and cooperate with all customers (internal and external)  
  • Assigns and supervises the care given by nursing and ancillary staff
  • Assists with the orientation of new nursing staff
Requirements:

A minimum of an Associate's degree (A.A.) in nursing from an accredited school is required.

3 years experience required.

To perform this job successfully, an individual should have knowledge of order processing systems.  This position requires a current, active Illinois Nursing license and current CPR certification.

Current Illinois Registered Nurse License and current certification in CPR.

 Additional InformationWeekend and/or Holiday Rotation may be required.
